The Chenango Health Network’s Community Health Worker provides Case Management support with a client centered, non-judgmental, trauma informed harm reduction approach to anyone who lives with Opioid Use Disorder, Substance Use Disorder and Alcohol Use Disorder. Our service program allows us to work with anyone in Chenango, Delaware and Otsego counties.
The Community Health Worker is part of the Rural Health Access Consortium (RHAC), a collaborative network of entities that operate across the healthcare, public health, population health, social services and education sectors with the primary goal of providing evidence-based, harm reduction-focused services. With the mission of becoming a leading healthcare and social services network, RHAC aims to identify best practices for engaging and delivering care to individuals who are at risk for, have been diagnosed with, and/or are in treatment and/or recovering from SUD/OUD, their cargeivers/families, and the members of the rural communities they live in. RHAC is committed to addressing the prevalence, morbidity, and mortality of Substance Use and Opioid Use Disorders (SUD/OUD) in the rural service area by providing key services in the areas of SUD/OUD prevention, treatment and recovery.
